Re-Entry Process

Any student who is hospitalized or takes a medical leave of absence while enrolled at MassBay must meet with Student Development for a re-entry conversation before resuming their studies. Students cannot resume coursework or register for future courses until this meeting occurs.

Initiating the Re-Entry Process

The above two forms are required before scheduling your medical re-entry conversation:

  1. Healthcare Provider Medical Re-Entry Form: Print this form, have it completed by your healthcare provider.  To submit this form please include in the student medical re-entry form, or have your medical provider send it to us directly.
  2. MassBay Student Medical Re-Entry Form: Submit this form online If additional information is needed, we will contact you via your MassBay email. Once we have all the required documentation, we will email you to schedule your re-entry conversation.

After the forms have been submitted, please contact Student Development at 781.239.3142 to schedule your re-entry meeting.

Questions & Answers

When the College is informed of a student’s hospitalization, a medical hold is placed on the student’s registration, pending the re-entry conversation. Confidentiality is maintained throughout the process.

Once a student feels well enough to return to classes, they must fill out the Student Medical Re-Entry Form and have their healthcare provider complete the Healthcare Provider Medical Re-Entry Form. The re-entry conversation can be scheduled only after both forms are submitted.

Any licensed healthcare provider familiar with your case, such as your primary care provider, hospital staff, psychiatrist, or psychologist, can complete the form.

Re-entry conversations are conducted virtually or face-to-face and include the student, a representative from the Dean of Students office, and a representative from MassBay counseling services. The meeting, typically 30 minutes long, is intended to review resources and support the student's return to classes, not to create barriers.

Students may have someone join them for the conversation, but they must complete a release form for each individual. The student must actively participate in the dialogue. Access the FERPA Release Form here and submit it to the Student Development staff member assisting you.

The College recognizes that a student’s physical, behavioral, and emotional health is an integral component of a student’s academic success at the College. The purpose of the protocol is to protect the health and well-being of all students by having procedures in place to identify, assess the risk of, intervene in, and respond to suicidal behavior. The policy is activated when the College has actual knowledge that a student is actively engaged in suicidal behavior, has previously engaged in suicidal behavior while enrolled at the College or recently before matriculation, or has stated plans or intentions to commit suicide. A student who engages in any of these behaviors may be required to comply with the College’s Re-Entry Policy before being permitted to resume classes. The Student Suicide Prevention Protocol is administered by the Dean of Student’s Office.
